The Enhanced Nutritional Optimization in LVAD Trial is studying how a special nutritional drink called Ensure Surgery can help heart failure patients who are about to have a left ventricular assist device (LVAD) implanted. This trial is looking at whether drinking this nutritional shake before surgery can improve the gut bacteria in these patients, reduce inflammation, and ultimately lead to fewer complications after surgery, such as infections or longer stays in the hospital. Participants will be divided into different groups to see how different amounts of Ensure Surgery affect their health both before and after the surgery.
To be eligible for this trial, participants must be over 18 years old, currently hospitalized, and scheduled to receive LVAD therapy. Some patients may not be able to join if they have other serious health conditions or if their surgery is happening within five days. Those who join the trial will drink the nutritional shake for several days before their surgery and will have some blood and stool samples taken at different times to help the researchers understand the effects of the supplementation.

- Inclusion Criteria:
- • age \>18 years
- • hospitalized
- • undergoing LVAD therapy (enrolled at time of acceptance)
- Exclusion Criteria:
- • intubated
- • congenital heart disease
- • infiltrative cardiomyopathy
- • unable to tolerate oral nutrition
- • surgery expected in \<5 days
