This clinical trial is studying the best way to treat a specific type of shoulder injury known as a supraspinatus tear, especially focusing on the long head of the biceps tendon, which is often involved in shoulder pain. Researchers want to compare two common surgical methods: tenotomy, where the tendon is cut, and tenodesis, where the tendon is repositioned. They are trying to find out which option leads to better recovery and less pain for patients, particularly looking at different age groups and activity levels.

To participate in this trial, you need to be between 50 and 80 years old and have a complete tear of the supraspinatus tendon, while also having an intact subscapularis tendon. You will be asked to consent to participate, complete some questionnaires, and attend follow-up visits after your surgery. If eligible, you can expect to receive specialized post-operative care and physical therapy to help with your recovery. It's important to note that those with previous biceps tears, certain infections, or who may have difficulty following the study procedures will not be included in the trial.

Eligibility criteria

  • Inclusion Criteria:
  • Patient voluntarily consents to participate in the study and has the mental and physical ability to participate in the study, fill out subjective questionnaires, return for follow-up visits, and comply with prescribed post-operative physical therapy.
  • Full thickness tear of the supraspinatus tendon
  • Intact subscapularis tendon
  • Primary rotator cuff repair
  • Age 50-80
  • Exclusion Criteria:
  • Previous full thickness biceps tear
  • Infection and neuropathic joints
  • Known or suspected non-compliance, drug or alcohol abuse
  • Patients incapable of judgement or under tutelage
  • Inability to follow the procedures of the study, e.g. due to language problems, psychological disorders, dementia, contraindication for MRI scan etc.
  • Enrolment of the investigator, his/her family members, employees and other dependent persons
  • Patient declines to participate in study
