ClinConnect Summary
This clinical trial is looking at how to improve care for patients who have multiple chronic conditions, also known as multimorbidity. Many people with multimorbidity struggle with various health issues, which can make managing their health very challenging. They often have to visit different doctors, take multiple medications, and follow complicated treatment plans, all of which can lead to stress and a lower quality of life. The trial will test a new approach that involves longer consultations with healthcare providers and better teamwork between different healthcare services to see if this helps improve patients' health and their overall experience with the healthcare system.
To participate in this trial, you'll need to be at least 18 years old and have two or more chronic conditions. You should also have been in contact with a specialist doctor in the past year and be taking at least five different prescription medications. If you join the trial, you can expect to receive more personalized care and support tailored to your specific health needs. This study is currently recruiting participants across Denmark, and the goal is to find effective ways to help those with complex health issues lead happier, healthier lives.
