This clinical trial is looking to understand the challenges and supports that African American adults with diabetic kidney disease face in managing their health. The goal is to create a special program that helps improve their health outcomes, quality of life, and self-care habits. The study is focused on people living in inner-city areas who might be dealing with multiple difficulties in life, known as multidimensional adversity.
To participate, individuals need to be at least 18 years old, identify as African American or Black, and have a diagnosis of type 2 diabetes and chronic kidney disease. They also need to show that they are facing at least one type of hardship as identified by a specific screening tool. Participants will not be able to take part if they have serious cognitive issues, active mental health conditions, or substance abuse problems. Those who join the study can expect to share their experiences and help shape a program designed just for them, aimed at improving their health and well-being.

Eligibility criteria
- Inclusion Criteria:
- • 1. self- report as AA/Black
- • 2. age ≥18
- • 3. screen positive for 1 or more adversities using the Centers for Medicare and Medicaid Services Accountable Health Communities Health-Related Social Needs Screening tool
- • 4. self-reported type 2 diabetes and self-report or screen positive for chronic kidney disease (CKD)
- • 5. able to communicate in English.
- Exclusion Criteria:
- • 1. cognitive impairment at screening visit
- • 2. active psychosis
- • 3. active alcohol or drug abuse/dependency
