This clinical trial is studying how well a person's kidney function can recover after they receive a simultaneous liver and kidney transplant (often referred to as an SLK transplant). The researchers want to find out how kidney health improves in patients who have kidney problems but are not on dialysis. They will be looking at specific measurements of kidney function and blood tests to help predict which patients are more likely to experience better recovery after the transplant.
To participate in this study, candidates must be over 18 years old and listed for both liver and kidney transplants, with two native kidneys and not currently on dialysis. Patients who are unstable, have had previous organ transplants, or are pregnant will not be eligible. If you join the study, you can expect to undergo tests that assess your kidney function before and after the transplant, and your participation will help researchers understand how to improve outcomes for future transplant patients.

Eligibility criteria
- Inclusion Criteria:
- • 1. \> 18 years old
- • 2. Listed for Liver Transplant for ESLD and Kidney Transplant (not on dialysis at time of transplant surgery)
- • 3. Candidates with two native kidneys.
- • 4. Willing to participate and sign informed consent form.
- Exclusion Criteria:
- • 1. Unstable patients (e.g. in the ICU or those who are on Continuous Renal Replacement Therapy (CRRT) who cannot complete the nuclear medicine scan or those with AKI on Hemodialysis
- • 2. Prior history of any solid organ transplant.
- • 3. Those who are currently pregnant or breast feeding.
- • 4. Patients who have Autosomal Dominant polycystic kidney disease
