This clinical trial, called the Care Coordination Educational Intervention Study, is looking at how teaching patients about cancer care and self-advocacy can improve their experience with care coordination. The study focuses on patients living in rural areas who have early-stage cancer, specifically breast, lung, or colon cancer, and who are about to start additional treatment known as adjuvant therapy. Participants will be divided into two groups: one group will receive educational support right away, while the other will receive it after four months of treatment. Researchers hope to see if this education helps patients feel more informed and supported in their care.
To take part in the study, you need to be at least 18 years old, have a diagnosis of stage I, II, or III breast, lung, or colon cancer, and live in a rural area of Vermont (not in Chittenden County). You should also be starting your chemotherapy within the next month. Throughout the study, participants will answer questions about their knowledge of cancer care and how well they feel their care is being coordinated. This trial is currently recruiting participants, and being part of it may help improve support for yourself and others in similar situations.

Eligibility criteria
- Inclusion Criteria:
- • 18+ years of age
- • Breast, lung, or colon cancer diagnosis
- • Stage I, II or III cancer
- • Adjuvant chemotherapy started \< 30 days or planned within the next 30 days
- • Rural residence location in Vermont (outside Chittenden County)
- • Proficient in English
- Exclusion Criteria:
- • History of prior malignancy treated with chemotherapy in last 3 years
- • Known or suspected neuro-cognitive impairment
- • Resident of Chittenden County, Vermont
- • Patients with current diagnosis other than stage I, II or III breast, colon or lung cancer
