This clinical trial is studying how reducing certain medications, known as anticholinergic and sedative medications, can affect physical function in older adults. These medications, which include some antidepressants and sleeping pills, can sometimes make it harder for people to think clearly and move around safely. The goal of the study is to find out how lowering the amount of these medications can help improve balance and mobility in older adults aged 65 and older who have a specific score indicating they may be taking too many sedative medications.
To be eligible for this trial, participants must be 65 years or older, live in the community (not in a care facility), and have a score that shows they are using these medications. They will work with healthcare providers to gradually reduce their medication over time. Throughout the study, participants will wear devices that track their movement and balance in both lab settings and their everyday lives. The researchers will also check their thinking ability using simple tests. This trial is important as it aims to better understand how managing medication use can enhance the quality of life and physical abilities for older adults.

Eligibility criteria
- Inclusion Criteria:
- • 65 years and older
- • community-dwelling
- • Drug Burden Index score of ≥ 1
- • Agree with a deprescribing plan targeting a reduction in Drug Burden Index score of ≥ 0.5.
- Exclusion Criteria:
- • Dementia
- • Need of a walker as a mobility assistive device
- • Unstable medical condition (≥ 1 unplanned hospitalization or ≥ 2 emergency department visits in the past month).
