This clinical trial is studying how certain body functions, known as autonomic functions, might affect blood pressure and heart stability during a specific procedure called per-oral endoscopic myotomy (POEM) for treating achalasia, a condition that makes it hard to swallow. The researchers believe that patients who have problems with these functions may experience more fluctuations in their blood pressure and heart rate during the surgery compared to those who do not. The study will involve tests to check autonomic function before surgery and monitor vital signs during the procedure.
To participate in this trial, patients must be at least 19 years old and scheduled for the POEM procedure at Gangnam Severance Hospital. However, those who are unstable in terms of blood pressure before surgery or cannot undergo a specific heart test will not be eligible. Participants can expect to undergo some tests before their surgery and have their heart and blood pressure closely monitored during the procedure. This study is currently recruiting participants and aims to gather important information that could help improve care for patients undergoing this treatment.

Eligibility criteria
- Inclusion Criteria:
- • 1. Patients who are scheduled to undergo per-oral endoscopic myotomy in Gangnam Severance Hospital
- • 2. Patients aged ≥ 19 years
- Exclusion Criteria:
- • 1. Patients who are hemodynamically unstable before surgery
- • 2. Patients in whom preoperative heart rate variability (HRV) test cannot be conducted
- • 3. Pregnant women of breastfeeding women
- • 4. Patients unable to read the informed consent form
