This clinical trial is studying how to better understand and manage patients with complex health issues treated at internal medicine clinics. It aims to identify important factors that might show how severe a patient's condition is and how active their disease may be. The researchers are looking particularly at patients with respiratory diseases, immune-related disorders, infections, metabolic issues, and other critical health problems. By gathering this information, the goal is to improve how these patients are diagnosed, monitored, and treated.
To be eligible for this study, participants should be receiving treatment at San Raffaele Hospital's internal medicine departments and have at least one of the following conditions: an immune-mediated disorder, a respiratory disorder, a metabolic disorder, sepsis (a serious infection), a rare disease, a pregnancy-related issue, or show signs of frailty. Participants should expect to share their health information and may undergo various assessments to help researchers gather the necessary data. This study is currently recruiting, and it's open to all genders, primarily focusing on adults aged 65 and older. Eligibility criteria
- Inclusion Criteria:
- • Receving treatment at internal medicine departments (inpatient, outopatient, day hospital) of San Raffaele Hospital
- * at least one suspected or confirmed diagnosis among:
- • 1. immune mediated disorder
- • 2. respiratory disorder
- • 3. metabolic disorder
- • 4. sepsis
- • 5. rare disorder (according to Italian Ministry of Health list)
- • 6. pregnancy-related disorder
- • 7. frailty defined as: either ≥2 CIRS (Cumulative Illness Rating Scale) Severity Index 10 or ≤70% Karnofsky scale
- Exclusion Criteria:
- • refusal to participate
- • enrolment in other intervantional study
