This clinical trial is studying a new approach to caring for patients with chronic diseases. To participate in this trial, individuals must be between the ages of 65 and 74 and have a chronic condition, such as heart disease, diabetes, or epilepsy, that is being managed by an APN. They also need to be part of a social security plan and must agree to participate after being informed about the study. Participants can expect to receive personalized care from the APN, who will help them develop skills to manage their health on a daily basis. This trial is currently recruiting participants, and it aims to provide a more effective way to support those living with chronic illnesses.

Eligibility criteria
- Inclusion Criteria:
- • Followed by APN, within the framework of an organizational protocol established with a patient's referring physician, for the management of one or more chronic pathology(ies) from the following list: stroke; chronic arterial disease; heart disease, coronary artery disease; type 1 diabetes and type 2 diabetes; chronic respiratory failure; Parkinson's disease; epilepsy
- • Affiliated or entitled to a social security plan
- • Having received informed information about the study and having co-signed, with the investigator, a consent to participate in the study
- Exclusion Criteria:
- • - Patient not referred by a physician for APN follow-up
