This clinical trial is studying ways to prevent Rheumatic Heart Disease (RHD) in Northern Uganda. RHD is a serious condition that can occur after a strep throat infection, causing damage to the heart. The researchers want to see if a community-based program can effectively help people at risk of developing this disease. They are currently looking for participants aged 18 to 26 who live in specific districts in Uganda and work at local healthcare centers.

- • providers at HCIIIs and HCIVs located in one of two involved Ugandan districts
