This study is testing whether using ultrasound during percutaneous dilatational tracheostomy (PDTS) improves safety and accuracy compared with the standard method that uses anatomy and bronchoscopy alone. Adults needing long-term mechanical ventilation are randomly assigned to either the standard PDTS or an ultrasound-guided PDTS (with bronchoscopic check). After the guidewire is placed, an independent doctor uses ultrasound to verify its position; if it’s not in the right place, it’s adjusted and the procedure is completed. The researchers will look at how often the guidewire goes through the thyroid isthmus (a potential complication) and at early problems such as bleeding or infection during the hospital stay, as well as whether ultrasound helps keep the guidewire in the correct position during the procedure.
Eligible participants are adults 18 and older who require long-term mechanical ventilation. The trial is being conducted at the Military University Hospital Prague, led by Charles University in the Czech Republic, and plans to enroll about 200 people. It is currently enrolling by invitation, with results not yet available and an estimated completion around 2025. There is no sharing of individual patient data outside the study.
