This clinical trial is studying two different surgical treatments for a specific type of fracture that can occur around a knee replacement, known as a distal femur fracture. The researchers want to compare two methods: one called Open Reduction and Internal Fixation (ORIF), which involves realigning and securing the broken bone, and another called Distal Femur Replacement (DFR), where the damaged part of the bone is replaced with an artificial joint. The goal is to see which method helps patients recover better and faster.

To participate in this study, you need to be at least 55 years old and speak English. You must have a specific type of fracture that needs surgery and be healthy enough for the procedure. However, if you have an active infection in your knee or can't undergo surgery for any reason, you wouldn't be eligible. If you join the trial, you can expect to undergo one of the two surgical treatments and be monitored for how well you recover, including your mobility and overall knee function. This research aims to improve care for patients with these challenging fractures.

Eligibility criteria

  • Inclusion Criteria:
  • English speaking patients
  • 55 years or older with Su Type II or Su Type III periprosthetic femur fractures or OTA/OA 33C or 338/3 native knee distal femur fracturing requiring surgical intervention and are medically fit to undergo surgical intervention
  • Exclusion Criteria:
  • Patients with an active total knee prosthetic infection
  • Patients unable to undergo surgical intervention
  • Patient with an open fracture
  • Non-English-speaking patients
  • Oncologic/pathologic fracture
  • Poly-trauma patient (or other associated major orthopaedic injuries)
