This clinical trial is studying a new way to improve care for older adults who are on dialysis, a treatment that helps when the kidneys are not working well. The goal is to make the care model better by getting input from patients, families, and healthcare providers, and then testing this improved approach in a small initial study. This will help researchers plan a larger study in the future.
People who might be able to join are adults aged 55 and older who live in the community (not in nursing homes) and are receiving dialysis. The study also includes those who may have some memory or thinking difficulties, but it does not include people with very advanced dementia, those who live in nursing homes, or those receiving hospice care. Participants will be asked to follow the new care approach being tested, and their experiences will help researchers understand if this model works well and how it might be improved. The study is not yet enrolling participants but will include people who speak English or Spanish.

- Patient Inclusion Criteria:
- • community-dwelling dialysis patients 55 and older
- • may have cognitive impairment
- Patient Exclusion Criteria:
- • advanced dementia
- • non-English or Spanish speaking
- • nursing home residents
- • hospice enrollees
