This clinical trial is studying how effective two different tests are in predicting the risk of death in critically ill patients who are experiencing low blood pressure due to conditions like sepsis or shock. The researchers will measure levels of lactic acid and a hormone called renin in the blood at several points during the patients' hospital stay. By comparing these measurements, the study aims to find out which test gives better information about a patient’s chances of recovery.
To participate in this study, patients should be between 21 and 60 years old, have been receiving medication to support their blood pressure for at least six hours, and be expected to stay in the hospital for more than 24 hours. Patients who are on kidney dialysis, taking certain heart medications, or who do not want to participate will not be included. Participants can expect to have their blood tested at the start of the study and then again over the next few days. This research could help doctors better understand how to care for critically ill patients in the future.

Eligibility criteria
- Inclusion Criteria:
- • 1. Patients aged 21-60 years old
- • 2. Those receiving vasopressors for greater than or equal to 6 hours to maintain a mean arterial pressure (MAP) ≥65 mm Hg.
- • 3. Anticipated stay \>24 hours.
- Exclusion Criteria:
- • 1. Patients or families refusing to participate in the study.
- • 2. Patients on renal replacement therapy.
- • 3. Patients receiving betablockers
