This clinical trial is studying the connection between diabetes and a dental condition called apical periodontitis, which occurs when infections develop at the root of a tooth. Researchers want to understand how these two conditions affect each other by measuring specific markers in the body that indicate inflammation. The study will compare how well the teeth heal in patients with and without diabetes after they undergo root canal treatment, which is a common procedure to save infected teeth.
To participate, you need to be between 30 and 55 years old, have at least 20 natural teeth, and have a specific dental issue confirmed by tests. The study is particularly looking for people with type 2 diabetes as well as healthy individuals who match them in age, gender, and body weight. If you join the trial, you’ll receive dental care and contribute to important research that could help improve treatment for both diabetes and dental health. It's important to note that some individuals, such as those with certain dental diseases or health conditions, won't be eligible to participate.

Eligibility criteria
- Inclusion Criteria:
- • Patients 30-55 year age group with minimum 20 natural teeth, with pulp necrosis confirmed by cold test and electric pulp test and radiographic evidence of periapical radiolucency with PAI score of ≥3 (24) or more in at least one mature tooth.
- • control in the systemically healthy group will be selected to match participants in terms of age, sex, and BMI.
- • control for participants with type 2 DM will be selected to additionally match with glycaemic control, drugs and dietary measures apart from other physical attributes.
- Exclusion Criteria:
- • Patients with clinical diagnosis of moderate to severe periodontitis (≥6 sites with probing depth ≥5 mm at separate teeth, with attachment loss and radiographic alveolar bone loss), Patients with immunosuppression, pregnancy, conditions requiring prophylactic antibiotics prior to dental treatment, bleeding disorders, medical conditions that may affect study participation (e.g., unstable cardiovascular disease) and chronic inflammatory conditions like rheumatoid
