This clinical trial is studying the use of methadone, a type of pain medication, to help control pain after orthopedic surgeries like hip and knee replacements. The researchers want to see if giving methadone during the surgery can provide better pain relief in patients who cannot receive other types of pain control methods, such as local anesthesia. This is important because many patients experience significant pain in the first day after surgery, and methadone may help manage that pain effectively without increasing the risk of side effects.

To be eligible for this trial, participants need to be between 18 and 80 years old and scheduled for elective hip or knee surgery. They should be generally healthy (ASA status 1-3), meaning they have mild to moderate health issues but are not severely ill. However, those with certain medical conditions, allergies to the medications being tested, or who are pregnant or breastfeeding cannot participate. If you join the study, you will receive either methadone or another pain medication during your surgery and be monitored for pain relief and any side effects for 24 hours afterward. This trial aims to find a better way to manage postoperative pain for patients who might struggle with traditional pain management options.

Eligibility criteria

  • Inclusion Criteria:
  • Age between 18 and 80 years old
  • ASA 1-3 status
  • Elective partial or total hip and knee arthroplasty
  • Exclusion Criteria:
  • Patient refusal
  • Preoperative renal failure (serum creatinine \> 2 mg/dL or 1.5-fold
  • Increase in basal plasma creatinine or GFR \< 90 ml/min/1.73m2)
  • Significant hepatic dysfunction (PT \<50% or increase in 3 times basal transaminases)
  • Known heart failure
  • Preoperative hemodynamic instability (preoperative use of inotropes or vasopressors)
  • Known methadone or sufentanil allergy
  • Preoperative opioid use or history of opioid abuse
  • Pregnancy and breastfeeding.
