This clinical trial is investigating two different types of reverse shoulder surgery, specifically looking at how the position of the shoulder joint affects recovery and muscle strength. The surgeries being studied have different centers of rotation—one is more inward (medialized) and the other is more outward (lateralized). The goal is to see which approach leads to better function and fewer complications for patients suffering from shoulder problems caused by rotator cuff issues or severe arthritis.
To be eligible for this trial, participants need to be at least 65 years old and have shoulder pain that hasn't improved with other treatments for at least six months. The trial is open to both men and women. Those who have had certain previous shoulder surgeries or specific bone issues may not qualify. If you join the trial, you can expect to undergo the surgery and then be monitored to assess your recovery and muscle performance over time. This research aims to help doctors better understand which surgical approach might work best for patients like you.

Eligibility criteria
- Inclusion Criteria:
- • 1. Male or female patients over 65 years of age with rotator cuff arthropathy refractory to conservative treatment after six months.
- • 2. Ability to understand the information sheet, informed consent and evaluation scales.
- Exclusion Criteria:
- • Patients with sequelae of fractures, rheumatoid arthritis, avascular necrosis, primary glenohumeral arthrosis, revision surgeries that have required conversion to a ATHI
- • 3. Patients in whom another surgical procedure has been associated with ATHI such as a tendon transfer
- • 4. Glenoid bone defects in the horizontal plane (Type B2 and C of the Walch classification) or in the vertical plane (E3 Sirveaux).
