This clinical trial is studying a new program designed to help women who have had high blood pressure during pregnancy, known as Hypertensive Disorders of Pregnancy (HDP), after they give birth. The program focuses on three main areas: teaching women to monitor their blood pressure using a mobile app, providing personalized support for managing their blood pressure, and helping them connect with their regular doctors for ongoing care. By enrolling 300 women starting the day after they give birth, the researchers hope to see if this comprehensive approach can improve blood pressure control and reduce health risks, especially for Black women.
To participate in this trial, women need to be over 18 years old and have been diagnosed with a hypertensive disorder related to pregnancy, such as preeclampsia or gestational hypertension. Participants can expect to use the app to report their blood pressure, receive guidance from a program navigator, and attend follow-up visits to monitor their health. The study aims to learn more about how effective this program is and whether stricter blood pressure management leads to better health outcomes for mothers.

- • Inclusion Criteria
- • 1. Age \>18 years
- • 2. Diagnosed with a Hypertensive Disorder of Pregnancy (HDP), which includes chronic hypertension, preeclampsia, gestational hypertension, HELLP Syndrome (Hemolysis, Elevated Liver enzymes, Low Platelets), or eclampsia (O10, O11, O13, O14, O15, O16) Exclusion Criteria
- • 1. Not able or willing to receive electronic surveys 2. Deemed inappropriate for study enrollment by the bedside nurse 3. Non-English speaking 4. Contraindication to ACOG-recommended hypertension control (i.e. chronic kidney disease, stroke)
