**Summary of the Clinical Trial: High Intensity Interval Training and Muscle Adaptations During Pulmonary Rehabilitation**
This clinical trial is exploring whether a specific type of exercise called High Intensity Interval Training (HIIT) can help patients with Chronic Obstructive Pulmonary Disease (COPD) who struggle with low oxygen levels during exercise. While pulmonary rehabilitation (PR) usually helps improve breathing and quality of life for many people with COPD, some do not see significant benefits, especially those who experience a drop in oxygen levels while exercising. The researchers believe that HIIT, which involves short bursts of intense exercise followed by rest, might be more effective in improving muscle function and increasing exercise capacity in these patients compared to traditional continuous exercise training.
To participate in the trial, individuals must be diagnosed with COPD and not have had any flare-ups in the past month. They should also experience a drop in oxygen levels during exercise tests. Participants will engage in HIIT during pulmonary rehabilitation sessions and will be monitored for changes in muscle function and levels of oxidative stress, which refers to a kind of stress on the body's cells. The goal is to determine if HIIT is both safe and beneficial for patients who typically find less improvement with standard rehabilitation methods. Eligibility criteria
- Inclusion Criteria:
- • COPD diagnosed according to Global Obstructive Lung Diagnosis criteria; no exacerbation of COPD in the last month
- • Referred for a pulmonary rehabilitation program
- • Exercise-induced oxygen desaturation during a CardioPulmonary Exercise Test, defined as a decrease of oxygen saturation by pulse oximetry \>4% from resting value.
- • Written informed consent
- • Negative pregnancy test whenever applicable.
- Exclusion Criteria:
- • Acute exacerbation of COPD during the last 4 weeks
- • Physical or psychological inability to perform exercise tests
- • Life-threatening cardiac events contra-indicating exercise training
- • Anticoagulant treatment (muscle biopsy)
- • Pregnant or breastfeeding womenFemmes
- • Subject with curator, deprived of freedoms or in safeguard of justice
- • Non covered by the french social security system
