**Summary of the Endogenous Pain Modulation Study**
This clinical trial is looking into why some patients continue to feel pain after having shoulder surgery for osteoarthritis, even when most people experience relief. The study will investigate if certain pain responses in the body, known as central sensitization and conditioned pain modulation (CPM), are present in patients undergoing total shoulder arthroplasty (TSA). Researchers will also consider how psychological factors and coping strategies might affect pain levels before and after surgery. By understanding these aspects, the study aims to improve care for patients with persistent pain.
To take part in this study, you must be 18 years or older and scheduled for shoulder surgery due to osteoarthritis. Participants will complete questionnaires and some tests to measure pain responses at three points: before surgery and then three and six months after. While there are minimal risks and no direct benefits from participating, your involvement could help improve future treatments for others in similar situations.

Eligibility criteria
- Inclusion Criteria:
- • Age ≥ 18 yrs
- • Scheduled to undergo primary total shoulder arthroplasty (TSA) or reversed shoulder arthroplasty (RSA) for primary shoulder osteoarthritis or cuff tear arthropathy
- • American Society of Anesthesiologists score 1, 2 or 3
- • Able to provide written informed consent
- Exclusion Criteria:
- • Regular use of anti-depressants or anti-epileptics for any purpose, including SNRIs and gabapentinoids
- • The presence of any chronic pain disorder other than osteoarthritis
- • Osteoarthritis in joints other than the affected shoulder, for which arthroplasty is/will be planned in the near future
- • Difficulty with or inability to perform psychophysical testing (eg. in case of cognitive or psychiatric disorders)
- • Difficulty with or inability to communicate with the investigators (eg. difficulty with the Dutch language, cognitive/memory disorders)
