Feasibility and Safety of In-bed Cycling/Stepping in Critically Ill Patients

This clinical trial is studying the safety and feasibility of a new rehabilitation method for critically ill patients in the intensive care unit (ICU). Patients often develop weakness due to their illness, which can make it hard for them to move around and perform daily activities after their time in the ICU. The researchers want to see if adding in-bed cycling and stepping exercises, along with standard rehabilitation techniques, can help these patients recover better.
To be eligible for the study, participants must be at least 45 years old, have been admitted to the ICU within the last 72 hours, and need to stay in the ICU for at least two days. They should also have some ability to move before getting sick. If they join the study, patients will either receive the new in-bed exercises plus regular therapy or just the regular therapy alone. The trial will track how many exercise sessions patients complete and how well they recover in terms of strength and daily activities. This study is important because it could lead to better ways to help critically ill patients regain their strength and improve their quality of life after their hospital stay.

Eligibility criteria
- Inclusion Criteria:
- • Age ≥ 45
- • Admission to ICU ≤ 72 hours
- • Patient deemed to need ≥ 48 hours of ICU care
- • Premorbid functional ambulation category≥ 2
- Exclusion Criteria:
- • Neurologic disorders i.Central nervous system: acute stroke, advanced dementia, hypoxic-ischemic encephalopathy ii. Peripheral nervous system: amyotrophic lateral sclerosis, myasthenia gravis, acute inflammatory demyelinating polyneuropathy
- • Acute deep venous thrombosis, pulmonary embolism
- • Pneumothorax
- • External fixator, superficial metallic implants, amputation, eschar, etc
- • Expected ICU discharge within 3 days of admission
- • Pregnant
- • Difficulty in obtaining consent (rejection, no family, if the family does not agree)
- • If the life expectancy is less than 6 months
