This clinical trial is exploring how a telehealth program can help older patients with certain blood cancers, such as acute myeloid leukemia and myelodysplastic syndromes, communicate better about their healthcare. The program focuses on advance care planning, which means helping patients think about and document their healthcare wishes for the future. The goal is to see if this telehealth approach can reduce anxiety and distress for patients, as well as encourage them to complete important forms like healthcare proxies and medical orders for life-sustaining treatment (MOLST).
To be eligible for this trial, participants need to be at least 60 years old and have a diagnosis of one of the specified blood cancers. They should also be receiving outpatient care, be able to understand and provide consent, and speak English. Caregivers and healthcare providers involved in the trial will also need to meet specific criteria. If you or a loved one qualifies, you can expect to participate in a virtual meeting where healthcare professionals will guide discussions about your care preferences and support you in making important decisions about your health. This study is currently recruiting participants, and it aims to improve communication and support for patients and their families during difficult times.

Eligibility criteria
- • Inclusion and Exclusion Criteria for Patients
- Inclusion criteria:
- • Age ≥60 years
- • A diagnosis of AML, MDS, and similar myeloid malignancies (including but not limited to MDS/MPN overlap syndrome, myelofibrosis)
- • Being managed in the outpatient settings
- • Able to provide informed consent
- • English-speaking
- Exclusion criteria:

- • Inclusion and Exclusion Criteria for Caregivers
- Inclusion criteria:
- • Age ≥18 years
- • Selected by patient when asked if there is a "family member, partner, friend, or caregiver with whom you discuss or who can be helpful in health-related matters"
- • Able to provide informed consent
- • English-speaking

- • Inclusion and Exclusion Criteria for Oncology Providers
- Inclusion criteria:
- • - Oncologists and/or APPs who will be conducting the telehealth-delivered ACP visit
