This clinical trial is looking at a new way to help manage heart failure using a system called the VitalCare platform. The goal is to see if remote patient monitoring can improve how patients engage with their health and reduce problems like hospital visits for heart failure. This is the first study of its kind that focuses on heart failure management using this specific platform, and it aims to understand how effective it can be for patients.
To participate in this study, you need to have a diagnosis of heart failure and experience certain symptoms. You should also have been hospitalized for heart failure within the last year and be comfortable using a smartphone or tablet. The trial is open to all genders and targets adults aged 65 to 74. If you meet these criteria and are interested, you will receive more information about what to expect during the trial, which is still in the planning stages and not yet recruiting participants. It's important to note that some people, such as those with certain heart devices or severe symptoms, may not be eligible to join the study.

Eligibility criteria
- Inclusion Criteria:
- • A clinical diagnosis of heart failure
- • New York Heart Association Class II or III congestive HF symptoms
- • Admission to hospital within 12 months for symptomatic heart failure.
- • Own a smartphone or tablet or comfortable with using one
- • English as primary language
- • Able to provide informed consent
- Exclusion Criteria:
- • Left Ventricular Ejection Fraction \< 35%
- • Implanted cardiac device (permanent pacemaker (PPM), implantable cardioverter defibrillator (ICD), chronic resynchronization therapy (CRT), implantable loop recorder (ILR))
- • CardioMEMS
- • Left ventricular assist device (LVAD)
- • NYHA Class I or IV congestive HF symptoms
- • Listed for cardiac transplant
- • Pregnant at time enrollment
- • End-stage renal disease
