This clinical trial is looking at ways to manage pain for patients undergoing total shoulder replacement surgery, also known as total shoulder arthroplasty (TSA). The goal is to find effective pain management strategies that can help patients recover more comfortably and possibly allow them to go home the same day after surgery. The trial will explore different methods of pain relief, including injections and medications, to see which approach works best for patients with conditions like arthritis or shoulder fractures.

To be eligible for this study, participants should be adults aged 18 to 80 who are scheduled for total shoulder replacement due to issues like osteoarthritis or rotator cuff problems. However, people with allergies to the study medications, certain sleep disorders, or those who have had previous shoulder surgeries won't be able to participate. If you join the trial, you can expect to follow a structured pain management plan and have regular check-ins for up to a year after your surgery to monitor your recovery.

Eligibility criteria

  • Inclusion Criteria:
  • Patients undergoing total shoulder arthroplasty for:
  • Osteoarthritis
  • Rheumatoid arthritis
  • Rotator cuff arthropathy
  • Acute fracture
  • Fracture sequelae.
  • Exclusion Criteria:
  • Patients with known allergy to study medications
  • Patients with obstructive sleep apnea
  • Prior TSA
  • patients previously on chronic narcotic medication
  • patients who are unable to read English
  • patients not able/willing to follow up for the study period of 12 months.
