This clinical trial is looking at how to improve diabetes care for Black and Hispanic adults who have type 2 diabetes and are struggling to manage their condition. The goal is to find out which types of support—like help from a pharmacist or a Community Health Worker—can help people better manage their diabetes and take their medications as prescribed.
To be eligible for the study, participants need to be between 18 and 90 years old, identify as Black or Hispanic, and be able to read or speak English or Spanish. They should also be currently taking diabetes medications and have a specific blood test result indicating their diabetes is not well controlled. Participants will be divided into four groups: one will receive standard care, while others will receive additional support from a pharmacist, a Community Health Worker, or both. Throughout the study, participants will need to attend training sessions and be available for follow-up. This research aims to find the best ways to support effective diabetes management in these communities.

Eligibility criteria
- Inclusion Criteria:
- • Men and women ages 18-90 with diagnosed type 2 diabetes who speak and understand English or Spanish.
- • Taking at least one oral or injectable diabetes medication
- • HbA1c ≥ 8% based on point of care test.
- • Will reside in the geographical area throughout the study period.
- • Have access to a phone during the study period.
- • Willing to attend all orientation/training sessions.
- Exclusion Criteria:
- • Having a caregiver who is the main decision maker in self-management.
- • Participating in another lifestyle, or medication adherence program.
- • Participated in standard MTM/MTM-related intervention in the last 6 months.
