This clinical trial, called "MDT Conferences for People With Diabetes and Multimorbidity," is studying how regular meetings between different types of doctors can help patients who have diabetes along with other chronic health conditions, like heart, kidney, or lung problems. The main goal is to see how these meetings can be effectively used in hospitals and what benefits they bring to both patients and doctors.

If you have diabetes and another chronic condition, you may be eligible to participate in this study. You'll be asked if you want your health case discussed in these doctor meetings, and to answer some questions about your well-being before the meeting and again six months later. Your information will be used to help researchers understand how these meetings can improve care. There are no specific exclusions, so if you're an adult receiving diabetes management, you could be a good fit for this study.

  • Inclusion Criteria:
  • Adult, any type of diabetes, diabetes management by general practitioner or Steno Diabetes Centre Odense, minimum one chronic condition within heart, kidney or lung specialities.
