If you have diabetes and another chronic condition, you may be eligible to participate in this study. You'll be asked if you want your health case discussed in these doctor meetings, and to answer some questions about your well-being before the meeting and again six months later. Your information will be used to help researchers understand how these meetings can improve care. There are no specific exclusions, so if you're an adult receiving diabetes management, you could be a good fit for this study.

- Inclusion Criteria:
- • Adult, any type of diabetes, diabetes management by general practitioner or Steno Diabetes Centre Odense, minimum one chronic condition within heart, kidney or lung specialities.
