This clinical trial is studying a new way for nurses to check for delirium, which is a sudden change in mental state that can happen in older adults, especially those with dementia. The goal is to see if daily screenings can help improve the care and outcomes for hospitalized older patients. The trial will take place in three hospitals in Pennsylvania and Massachusetts, focusing on patients aged 70 and older who are expected to stay in the hospital for at least three days.
If you or a family member are eligible and choose to participate, you will undergo screenings for delirium on the first three days of your hospital stay, and you will also be contacted a month later to discuss your health experience. Care partners, like family members or friends who help care for you, will also be interviewed about their experiences. The study aims to find out how effective these daily checks are in reducing complications like falls and medication side effects, as well as how they affect both patients and their caregivers.

- Inclusion Criteria:
- • General medicine (hospitalist) service
- • Age 70 or older
- • Expected hospital length of stay of 3 or more days
- Exclusion Criteria:
- • Inability to communicate adequately in English
- • Admitted for terminal care
