This approach could make it easier for you to manage your health without frequent long visits to the hospital.

- Inclusion Criteria:
- • Looked after by a clinical care team at Manchester University NHS Foundation Trust
- • Given consent
- • Have received a lung transplant
- Exclusion Criteria:
- • Consent not given
- • Not able to operate the remote monitoring platform
