The SEISMIC-R trial is studying how different combinations of nursing staff in Intensive Care Units (ICUs) affect patient care and recovery. Recently, the way ICUs are staffed has become very important, especially after the pandemic. This research aims to find out which staffing models—meaning the number and qualifications of nurses—work best to help patients recover and improve survival rates. The study will look at six different hospitals across the UK and gather information through surveys, observations, and interviews with nurses, nurse managers, and patients or their families. By understanding what staffing combinations are most effective, the goal is to create a helpful guide that all ICUs can use.
To participate in this trial, individuals need to meet certain criteria. For example, registered nurses must have worked in an ICU for at least six months, while nurse managers and organizational leads need a minimum of one year in their roles. Patients or family members can also be involved if they are over 18 and have had recent experience in a general ICU. Participants can expect to share their insights and experiences, which will help shape better care practices in ICUs across the country. This research aims to improve patient outcomes and ensure that critical care is delivered safely and effectively.

Eligibility criteria
- • Inclusion Criteria: Interviews with nursing staff
- • Registered Nurses who have worked in ICU for at least six months. Inclusion criteria: Interviews with nurse managers
- • Clinically-based nurse managers who have worked in ICU for at least one year. Inclusion criteria: Interviews with organisational leads
- • Organisational leads who have been working in their role and in the ICU field for at least one year.
- • Inclusion criteria: Interviews with regional managers/commissioners • Regional managers/commissioners who have been working in their role and in the ICU field for at least one year.
- • Inclusion criteria: Interviews with patients/families
- • Patient or family member over 18 years old.
- • Patients who have been in General ICU for at least 48 hours in the last 6 months.
- • Family members who have visited ICU for at least 20 mins on two days in the preceding 6 months.
- • Patient discharged from hospital at least 2 weeks prior to the interview.
- • Patient expected to be well enough, after hospital discharge, to attend the interview and to have capacity to consent.
