This clinical trial is studying two different ways to manage pain after a surgical procedure called Laparoscopic Sleeve Gastrectomy (a type of weight-loss surgery) to see which method helps reduce the need for pain medication, specifically opioids. The researchers want to find out if a technique called the Transversus Abdominis Plane Block (which involves injecting a numbing medication called bupivacaine around the surgical area) is more effective than simply injecting the medication into the wound itself.
If you're between the ages of 18 and 75 and plan to have this surgery at Srinagarind Hospital, you might be eligible to participate. The trial will involve randomly assigning you to one of the two pain management methods. Participants can expect to be monitored for their pain levels and the amount of opioid medication they use after the surgery. It's important to note that individuals who are allergic to certain pain medications or who have a history of opioid addiction cannot participate. This study aims to improve pain management options for patients undergoing this type of surgery.

- Inclusion Criteria:
- • patient undergoing LSG at Sringarind hospital as indicated by Thai guideline
- Exclusion Criteria:
- • Allergic/contraindicated to bupivacaine, morphine, fentanyl, NSAIDs or Nefopam
- • Unable to describe pain score Opioid addict
- • denied consent
