The Mindful Steps 2.0 clinical trial is designed to help patients with Chronic Obstructive Pulmonary Disease (COPD) and Chronic Heart Failure (HF) become more physically active. The study will compare a new program called Mindful Steps to standard care to see if it can effectively increase daily step counts in people who are not currently active. About 136 participants will be involved, and they will either follow the Mindful Steps program or continue with their usual care for one year. Everyone will receive a helpful booklet called "Walking for Health" to guide them in their journey.
To be eligible for this trial, participants need to have a diagnosis of COPD or HF and must have medical approval to join an exercise program. They should also have access to a device that can run the Fitbit app and an active email account. It’s important to note that individuals who have had a recent worsening of their condition or cannot walk may not qualify. If you decide to join, you can expect to engage in activities that encourage more movement and potentially improve your health while being supported through this remote program.

Eligibility criteria
- Inclusion Criteria:
- • Clinical diagnosis of COPD (defined as cigarette smoking history \> 10 pack-years, and either a ratio of FEV1 to forced vital capacity \< 0.70 or chest CT evidence of emphysema) OR clinical diagnosis of HF syndrome (with left ventricular systolic dysfunction or preserved ejection fraction and New York Heart Association Class 1-3)
- • Medical clearance from provider to participate in an exercise program
- • Have an active email account; access to a computer device (desktop, laptop, tablet, or smartphone that supports the Fitbit app).
- Exclusion Criteria:
- • COPD or HF exacerbation in the previous month
- • Inability to ambulate
- • Inability to collect at least 7 of 14 days of baseline step counts
- • Current participation in a cardiac or pulmonary rehabilitation program
