This clinical trial is looking at a new program designed to help older African Americans who have type 2 diabetes that isn’t well controlled. The program, called the DM Social Needs Intervention, combines education about managing diabetes with support to address social challenges that might be affecting their health. The researchers will compare this program with standard care by enrolling 100 participants aged 50 and older. Half will receive the new intervention, while the other half will continue with usual care. The goal is to see if this new approach can improve health outcomes and patients' overall satisfaction.
To be eligible for this trial, participants must be at least 50 years old, identify as African American or Black, have been diagnosed with type 2 diabetes, and have a specific blood sugar level that indicates their diabetes is not well managed. Participants can expect to receive support through phone calls and education on managing their diabetes more effectively, while also helping to resolve any social issues that may be impacting their health. This study is currently recruiting, and it aims to find better ways to help older adults manage their diabetes and improve their quality of life.

Eligibility criteria
- Inclusion Criteria:
- • 1. Age 50 and older;
- • 2. self-identified as African American or Black;
- • 3. self-reported diagnosis of T2DM;
- • 4. HbA1C \>=8% at the screening visit.
- Exclusion Criteria:
- • 1. self-reported participation in other diabetes clinical trials;
- • 2. alcohol or drug abuse or dependency as assessed by the CAGE-AID;
- • 3. Mental confusion at screening assessment suggesting significant dementia;
- • 4. life expectancy \< 6 months at screening assessment.
