The BP-REACH study is looking at a special program designed to help people lower their blood pressure, particularly those who have had a heart attack or stroke. This program involves a team of healthcare professionals, including pharmacists and health coaches, who work together to provide support. The main goal is to find out if this team-based approach is more effective than standard care in helping patients lower their blood pressure over a year. The study will also explore if participants experience improvements in their overall health and quality of life.
To be eligible for this study, participants should be adults who have received care at specific healthcare centers in Los Angeles and have high blood pressure (130 mm Hg or higher). They should also be able to speak either English or Spanish. Participants will receive either the new team-based care or the usual treatment and will be followed for 12 months to see how well their blood pressure improves. If you or a loved one fit these criteria and are interested in participating, this study could provide valuable insight into better ways to manage blood pressure in patients at high risk for cardiovascular issues.

Eligibility criteria
- Inclusion Criteria:
- • Patients of LA General Medical Center, Rancho Los Amigos, Olive Medical Center or Harbor-UCLA Medical Center with ischemic stroke, hemorrhagic stroke, or myocardial infarction
- • English or Spanish speaking
- • Systolic blood pressure (SBP) ≥ 130 mm Hg
- Exclusion Criteria:
- • Age \< 35 years
- • SBP \< 130 mm Hg
- • Only speaks a language other than English or Spanish
- • Cannot provide informed consent due to dementia or aphasia
