ClinConnect Summary
This clinical trial, called "A Prescription for Health Equity," is exploring a new program that helps people with prediabetes or Type 2 diabetes improve their health through better access to fresh produce. The goal is to create a "produce prescription program" (PPR) where healthcare providers can prescribe fruits and vegetables to patients, making it easier for them to eat healthier. The study will work closely with community members to ensure the program meets their needs and addresses any barriers they face, such as low income or lack of access to healthy food.
To be eligible for this study, participants must be at least 18 years old, patients of Griffin Hospital or Griffin Faculty Physicians, and diagnosed with prediabetes or Type 2 diabetes. They should also be low-income and qualify for programs like SNAP (food assistance) or Medicaid. If you join the study, you'll help shape the program and receive support to improve your diet and health over six months. The researchers hope that by participating, you'll have better access to nutritious food, experience improvements in your health, and reduce your healthcare costs over time.
