This clinical trial is studying a new way to help people with neurologic disorders after they leave the hospital. The trial is comparing two types of aftercare: a tele-rehabilitation program, where patients receive support through video calls and online tools, versus a traditional paper-based program. The goal is to see if the tele-rehabilitation can help patients maintain the progress they made during their hospital stay and improve their ability to live independently at home. Researchers believe that this online support, combined with guidance from therapists, will lead to better outcomes for patients.
To join the trial, participants need to be 18 years or older and have challenges with movement, thinking, or speaking, but they should also be able to use a smartphone or computer, either on their own or with help. The trial is currently looking for participants, and those who take part can expect to engage in activities designed to help them practice skills they learned in rehabilitation, with the support of a therapist. It's important to know that individuals with severe difficulties in awareness or memory may not be able to participate safely, as determined by their doctors.

Eligibility criteria
- Inclusion Criteria:
- • aged 18 years or older,
- • experiencing problems with motor function, sensory function, cognition, swallowing or speech production (regardless of their diagnosis),
- • being insured with SVS,
- • agreeing to telerehabilitation after discharge,
- • having sufficient motor and cognitive abilities to operate a smartphone, tablet or PC, or being sufficiently supported by caregivers, and
- • being able to independently perform the targeted tasks assigned to them.
- Exclusion Criteria:
- • - pronounced impairments in awareness, memory, attention, spatial performance, executive performance, apraxia, speech, and visual perception that do not allow a safe study participation according to the treating physicians or therapists
