This clinical trial is studying how to manage infections in nursing home residents who have dementia, including Alzheimer’s disease. The researchers want to find out if using a special approach to care can help these residents use fewer antibiotics and avoid unnecessary medical procedures compared to the usual care they receive. This trial is currently recruiting participants, and it aims to improve the quality of care for people living with dementia in nursing homes.
To be eligible for this study, participants need to be at least 60 years old, diagnosed with any type of dementia, and have been living in a nursing home for more than 90 days. They should also have a cognitive functional score that indicates at least mild cognitive impairment. If someone joins the trial, they will receive either the new approach to infection management or the standard care currently provided. Overall, this research aims to make healthcare for nursing home residents with dementia safer and more effective.

Eligibility criteria
- Inclusion Criteria:
- • 1. Age \> 60
- • 2. A diagnosis of dementia (any type)
- • 3. Cognitive Functional Scale (CFS) \> 1
- • 4. NH length of stay \>90 days
- • The CFS score categorizes cognitive impairment status based on data in the electronic health record into: 1. None, 2. Mild, 3. Moderate, and 4. Severe (advanced). For the primary outcome, the analysis will be restricted to residents with a CFS score of 3 or 4.
- Exclusion Criteria:
- • 1. Less than 60 years of age
- • 2. Living in nursing home for less than 90 days
- • 3. Does not have diagnosis of dementia
- • 4. Does not meet CFS \>1 score
