This clinical trial is studying the effects of a medication called Tadalafil, which is commonly used to treat erectile dysfunction, in patients who have recently undergone surgery for prostate issues. The researchers want to find out if taking Tadalafil can help improve both urinary symptoms and erectile function in men who are experiencing difficulties after their surgery. The trial is open to men aged 40 and older who have specific urinary symptoms related to prostate enlargement and have not found relief from other treatments.
To participate, men must be sexually active and have certain urinary symptoms that are causing them problems, while not currently taking specific medications that could interfere with the study. If eligible, participants will take Tadalafil once a day and will be monitored for improvements in their symptoms. This trial aims to help understand how Tadalafil might support recovery after prostate surgery, which could be important for improving quality of life for many men facing these issues.

Eligibility criteria
- Inclusion Criteria:
- • Patients' age ≥ 40 years
- • LUTS secondary to BOO due to BPH who failed medical treatment
- • International prostate symptom scores (IPSS) \>15 and bother score (QOL) ≥ 3 (according to IPSS question 8)
- • Peak urinary flow rate (Qmax) \<15 ml/sec.
- • ASA (American society of anaesthesiologists) score ≤ 3.
- • Sexually active men, not receiving PDE5I
- Exclusion Criteria:
- • Patients using nitrates, potassium channel opener, or α1-blockers doxazosin and terazosin for control of hypertension
- • Patients with unstable angina pectoris, recent myocardial infarction or stroke myocardial insufficiency.
- • Patients with severe hepatic or renal insufficiency.
- • Patients who are sexually inactive.
- • Patients with uncontrolled diabetes mellitus or neurologic condition(eg. parkinsoism)
