This clinical trial is studying a new approach called PReTEE, which is a simpler way to perform a special heart scan known as transesophageal echocardiography (TEE). This scan helps doctors look at the heart more closely, especially during high-risk surgeries, to identify any heart problems that might make surgery more difficult. The trial aims to see how well this new technique works in helping doctors during a critical time when patients need to be carefully monitored.
To participate in this study, you need to be at least 18 years old and scheduled for a high-risk cardiac surgery, such as having a weakened heart or needing multiple heart valve repairs. However, if you have certain conditions like esophageal problems or are undergoing emergency surgery, you may not be eligible. If you join the trial, you can expect to have the TEE scan as part of your care, which could help improve the results of your surgery. This study is currently recruiting participants, and it's important to discuss any questions or concerns with your healthcare team.

Eligibility criteria
- Inclusion Criteria:
- • 1. Age of 18 yr and greater;
- • 2. High-risk cardiac surgery (one of the followings): Baseline left ventricular ejection fraction \< 50%, Coronary artery bypass graft combined with valve procedures, Multiple valve procedures (≥ 2), Aortic root or arch involved, Euroscore \> 6, Previous cardiovascular surgery
- Exclusion Criteria:
- • 1. Lack of patient consent;
- • 2. Esophageal pathology (stricture, tumor, perforation/laceration, ulcer or fistula, diverticulum);
- • 3. Hiatus hernia; Perforated viscus;
- • 4. Active/recent upper gastrointestinal (GI) bleed;
- • 5. Non-elective cardiac procedures;
- • 6. Preoperative mechanical cardiac support (ECMO, LVAD or IABP)
