This clinical trial is looking at how social networks, like those on smartphones and apps such as WeChat, can help people who are at high risk for stroke manage their medications and health better after leaving the hospital. The goal is to see if connecting with others can lead to better adherence to prescribed medications and improved control of health risks associated with strokes.
To participate in this study, you need to be at least 18 years old and have been hospitalized due to a high risk of stroke. You should also be able to use a smartphone and have a WeChat account. If you have a caregiver, they must also be able to use a smartphone. Participants will engage with others through these platforms to support each other in managing their health. It's important to know that if you have other serious health issues or can't use a smartphone, you may not be eligible for this study. Overall, this trial aims to harness the power of social connections to improve health outcomes for those at risk for strokes.

Eligibility criteria
- Inclusion Criteria:
- • Age ≥18 years
- • High-risk population of stroke for hospitalization
- • Modified Rankin Scale score≤ 2
- • Patients or primary caregiver have smart phone and wechat accounts
- • Patients take at least one drug for a long time (antihypertensive, hypoglycemic, lipid-lowering, anticoagulant, antiplatelet drugs)
- • Written informed consent
- Exclusion Criteria:
- • Patients and their families are unable to operate smartphones
- • Having other diseases that interfere with clinical follow-up assessment (such as cancer, dementia, severe mental illness, etc.)
- • Life expectancy is less than 12 months
- • Patients living in the absence of network conditions
