This clinical trial is studying how to give the right dose of a medicine called vancomycin to patients in the Intensive Care Unit (ICU) and orthopedic departments. Vancomycin is often used to treat serious infections, but in the past, only a small number of patients received the correct amount of the drug after their first dose. The goal of this study is to see if using a new, easy-to-use tool to personalize the dosage can help more patients reach the desired drug levels in their system. Researchers will look at how well this method works by checking the drug levels in patients at specific times after starting treatment.
To take part in this trial, patients must be adults receiving continuous vancomycin treatment in the ICU or orthopedic department and able to provide consent or have a legal representative do so. Unfortunately, pregnant women, children, and patients with certain medical conditions, such as those who have had organ transplants or are on specific life-support treatments, cannot participate. Those who do join the study can expect to have their vancomycin levels monitored closely to ensure they are getting the right amount for effective treatment.

Eligibility criteria
- Inclusion Criteria:
- • Adult patients who receive continuous vancomycin treatment at the ICU or orthopaedic department
- • Patient or their legal representative is able and willing to sign the Informed Consent Form
- Exclusion Criteria:
- • Pregnant woman
- • Children
- • Patients with a transplantation history
- • Patients on continuous renal replacement therapy (CRRT)
- • Patients receiving extracorporeal membrane oxygenation (ECMO)
