This clinical trial is focusing on understanding a heart condition called functional mitral regurgitation (FMR), which often occurs in patients with weakened heart function. The researchers want to find better ways to predict how patients might respond to treatments by using advanced imaging techniques called cardiac magnetic resonance (CMR). They plan to enroll 360 adult participants, aged 18 and older, who have reduced heart function and significant FMR. Participants will receive tailored medical treatments and will undergo follow-up imaging after six months to see how their condition is changing.
To be eligible for the study, participants need to have a specific level of heart function measured by CMR (less than 55%) and a certain degree of mitral regurgitation. Certain conditions, like severe valve problems or recent heart attacks, may exclude someone from participating. Those who join can expect to receive personalized care and monitoring, which may include treatments like the Mitraclip procedure if necessary. This trial aims to enhance our understanding and management of FMR, ultimately leading to better outcomes for patients with this condition.

Eligibility criteria
- Inclusion Criteria:
- • 1 CMR LVEF \<55% 2.FMR Fraction\>20% or echo criteria consistent with at least moderate mitral regurgitation with adequate image quality
- Exclusion Criteria:
- • 1. \>moderate aortic regurgitation/stenosis,
- • 2. \<18 years of age,
- • 3. acute myocarditis,
- • 4. eGFR\<15
- • 5. HCM
- • 6. cardiac amyloidosis/sarcoidosis
- • 7. prior mitral valve intervention
- • 8. myocardial infarction within 8 weeks of CMR
