ClinConnect Summary

This clinical trial, called the GDMT Research, Education & Assist Trial, is focusing on improving the use of important medications for patients with heart failure with reduced ejection fraction (HFrEF). HFrEF is a serious condition where the heart doesn't pump blood as well as it should, leading to higher risks of hospitalization and a lower quality of life. The goal of the study is to help more patients receive guideline-directed medical therapies (GDMT), which are proven to help improve survival and reduce heart-related issues.

To participate in this trial, individuals need to be at least 18 years old and have been seen at a Geisinger cardiology clinic for their heart condition. They must have a diagnosis of HFrEF or have a specific measurement of heart function showing their heart is not working well. However, people currently receiving hospice care, those with allergies to the medications being studied, or those already on all four types of these heart failure medications cannot participate. If eligible, participants will be part of a program designed to ensure they receive the best possible heart failure treatments and support.

Eligibility criteria

  • Inclusion Criteria:
  • Patients aged 18 years or older AND
  • Completed visit at included Geisinger cardiology outpatient clinics (office visit or telemedicine) AND
  • Clinicians are on a list of currently active Geisinger clinicians in outpatient cardiology clinics who can prescribe heart failure medications AND
  • Active problem list diagnosis of HFrEF at time of Cardiology clinic encounter OR Left Ventricular Ejection Fraction (LVEF) \< 40: most recent to the cardiology clinic encounter within 2 years of the visit.
  • Exclusion Criteria:
  • Currently in hospice or palliative care (ICD 10 code: Z51.5)
  • Patient is allergic to each category of GDMT
  • Patient is prescribed medications from all four categories of GDMT, including ARNI specifically
