This clinical trial is studying two different methods of helping patients breathe when they are on a machine called a ventilator. Specifically, it is comparing a standard method called Pressure Support Ventilation (PSV) with a newer method called Neurally Adjusted Ventilatory Assist (NAVA). The goal is to see which method helps patients wean off the ventilator more effectively, particularly those who have had trouble doing so in the past. Researchers will look at how well the diaphragm, the main muscle used for breathing, is working during these different methods.

To participate in this trial, patients must be between 65 and 74 years old and have tried to stop using the ventilator at least once without success. They should have been on the ventilator for at least 24 hours. However, pregnant women, those with obesity (a Body Mass Index over 35), or certain muscle diseases cannot participate. If eligible, participants will undergo tests that measure their breathing muscles while using either PSV or NAVA. This study aims to improve care for patients with respiratory failure by finding out which method helps them breathe better and recover faster.

  • Inclusion Criteria:
  • patients who have failed at least one weaning attempt
  • mechanical ventilation for at least 24 hours
  • Exclusion Criteria:
  • Pregnancy
  • Obesity (Body Mass Index \> 35 kg/m2)
  • Contraindication to the insert of a nasogastric tube
  • Neuromuscular diseases
