This clinical trial is studying how different types of anesthesia—desflurane, sevoflurane, and propofol—affect the likelihood of developing postoperative delirium in older patients after major noncardiac surgeries. Postoperative delirium is a condition that can cause confusion and memory problems in patients over 65, and preventing it is very important for their recovery. The researchers are especially interested in whether desflurane, which works more quickly than the other two anesthetics, can help lower the risk of this condition.
To participate in this trial, you need to be at least 65 years old and scheduled for an elective surgery that will take at least two hours. You must also be able to give written consent to join the study. However, if you have certain health conditions—like a history of dementia, certain muscle disorders, or if you're undergoing emergency surgery—you would not be eligible. Participants will receive one of the three types of anesthesia during surgery and will be monitored afterward to see if they experience delirium. This research could help doctors understand how to better care for elderly patients undergoing major surgeries.

Eligibility criteria
- Inclusion Criteria:
- • Provide written informed consent
- • ≥65 years of age
- • Scheduled for elective major noncardiac surgery with estimated time of surgery ≥ 2 hours
- Exclusion Criteria:
- • Patients undergoing emergency surgery
- • BMI \> 45 kg/m\^2
- • History of diagnosed dementia
- • Language, vision, or hearing impairments that may compromise cognitive assessments
- • History of malignant hyperthermia
- • History of structural muscle disease
- • History of organ transplantation (kidney, liver, lung, heart)
- • Patients undergoing hyperthermic intraperitoneal chemotherapy
- • ICU patients undergoing surgery
