This clinical trial is studying ways to manage pain after a specific type of lung surgery called video-assisted thoracoscopic surgery (VATS). After this surgery, many patients experience moderate to severe pain, which can affect their recovery and lead to complications. The trial aims to compare three different techniques that help control pain: cryoanalgesia, epidural anesthesia, and erector spinae muscle plane block. By finding out which method works best, the goal is to improve pain management for patients undergoing lung surgery.
To participate in this study, patients should be between the ages of 65 and 74 and scheduled for VATS anatomical pulmonary resection, which includes surgeries like lobectomy or segmentectomy for lung cancer or other lung diseases. However, certain individuals, such as those under 18, pregnant women, or those with specific health issues, may not be eligible. If you choose to participate, you can expect to receive one of the pain management techniques during your surgery and help us understand how well it works in reducing pain afterward. This trial is currently recruiting participants, and your involvement could lead to better pain control for future patients.

Eligibility criteria
- Inclusion Criteria:
- • all patients undergoing VATS anatomic pulmonary resection (lobectomy or segmentectomy) for benign or malignant disease
- Exclusion Criteria:
- • Refusal or inability to give informed consent to the study protocol
- • Age \< 18 years
- • Pregnancy
- • Allergies or contraindications to any of the treatments considered in the experimental design
- • Pre-operative use of pain medication for chronic pain or neuropathic pain
- • History of previous major chest surgery
- • American Society of Anesthesiologists (ASA) class \> 3
- • Need for intensive care unit stay in the post-operative period
- • Patients undergoing anatomical pulmonary resections different than lobectomy or segmentectomy (bi-lobectomy or pneumonectomy).
- • Patients undergoing bronchial and/or vascular resections and reconstructions
