This clinical trial is studying how different types of anesthesia can affect brain function after surgery in older patients who have a condition called normal pressure hydrocephalus. These patients will be undergoing a surgery called VP shunt surgery, which helps drain excess fluid from the brain. The researchers want to see if using a specific medication called dexmedetomidine, along with careful management of anesthesia during surgery, can improve brain function after the operation and reduce inflammation in the brain.
To participate in this study, patients should be over 60 years old, fluent in Chinese, and scheduled for the surgery. They should also expect to stay in the hospital for at least three days after their procedure. However, individuals with certain health issues, such as unstable heart conditions or severe liver or kidney problems, as well as those with specific mental health disorders, are not eligible to join. Participants will be closely monitored to see how these anesthetic methods impact their recovery and cognitive function, helping to improve care for future patients.

Eligibility criteria
- Inclusion Criteria:
- • patients with communicating hydrocephalus for elective VP shunt surgery under general anesthesia
- • age\> 60 y/o
- • conscious clear
- • fluency in Chinese
- • anticipated hospital stay ≥ 3 days after surgery
- Exclusion Criteria:
- • unstable preoperative condition (unstable angina, CHF, asthma attack) within 4 weeks before surgery,
- • severe hepatic dysfunction or renal failure
- • history of neuropsychological diseases such as schizophrenia, Parkinson's disease, dementia, stroke,
- • pre-op cognitive impairment,
- • preoperative delirium,
- • preoperative depression,
- • allergy to contrast medium of MRI.
