This clinical trial, called "Transitions of Care for Stroke Patients," is looking to improve how stroke patients recover after they leave the hospital. Researchers believe that starting rehabilitation right after a stroke can help patients regain their abilities and reduce the negative effects of being inactive. The trial is specifically testing whether better coordination between medical teams and rehabilitation specialists can lead to better recovery and follow-up care for these patients.
To participate in this study, individuals must be hospitalized at Robert Wood Johnson University Hospital for either Ischemic Stroke (a type of stroke caused by a blockage in a blood vessel) or Intracerebral Hemorrhage (a type of stroke caused by bleeding in the brain). Unfortunately, patients with certain other conditions, such as a Subarachnoid Hemorrhage (bleeding in the area between the brain and the tissues covering it) or those who have had a Transient Ischemic Attack (a temporary stroke-like event), are not eligible. If someone qualifies and joins the trial, they can expect to receive personalized care aimed at improving their recovery journey.

Eligibility criteria
- Inclusion Criteria:
- • Stroke patients hospitalized within Robert Wood Johnson University Hospital system for Ischemic Stroke or Intracerebral Hemorrhage.
- Exclusion Criteria:
- • Those not in the Robert Wood Johnson University Hospital system.
- • Those with a Subarachnoid Hemorrhage or Transient Ischemic Attack (\< 24 hours).
