This clinical trial is looking at two types of anesthesia—spinal anesthesia and saddle block—used during a common surgery called transurethral resection of the prostate (TURP). TURP is often performed on older men who have prostate enlargement, and it’s especially important to choose an anesthesia method that maintains stable blood flow and minimizes changes in heart function, especially for those with heart issues. Since general anesthesia can cause more fluctuations in blood pressure and heart rate, this trial aims to find out which regional anesthesia technique works best for elderly patients with heart problems.

To participate in this trial, men aged 65 and older who have certain heart conditions, like a history of heart issues or abnormal heart test results, may be eligible. However, those under 65 or with specific medical conditions that could complicate the procedure will not be included. Participants can expect to receive either spinal anesthesia or saddle block during their surgery and will be monitored closely for any effects on their heart and blood flow. Overall, this study is important for improving anesthesia care in older patients undergoing prostate surgery.

Eligibility criteria

  • Inclusion Criteria:
  • American Society of Anesthesiologists (ASA) III,
  • with Ischemic Heart Disease (history of Myocardial Ischemia,
  • a history of a positive treadmill test result (Electro Cardio Graph stress test),
  • use of nitroglycerin, chronic stable angina for more than two months, or an ECG with abnormal Q waves), with ejection fraction (EF) 35%-50%,
  • Exclusion Criteria:
  • patients under 65 years,
  • patients with any diseases that increase intraabdominal pressure (any intra-abdominal mass),
  • general contraindications of spinal anesthesia as patient refusal, coagulation disorders, local infection at the site of the block, psychiatric illness,
