The PRESAGE CARE trial is studying a new way to help older adults who have recently been hospitalized for heart failure stay healthy and avoid going back to the hospital. This study will use a smartphone-based tool that allows family caregivers to check in on their loved ones' daily lives by answering a few simple questions twice a week. The goal is to see if this system can help predict when someone might need to visit the emergency room or be hospitalized again, so that healthcare providers can step in early.
To be part of this study, participants need to be 65 years or older, live at home, and have been recently hospitalized for heart failure. They should also have a family member who can help by filling out the brief questionnaires. The study will last about six months, during which researchers will gather information on how well the tool works, how satisfied participants are, and whether it helps reduce hospital visits. This research is important because it could lead to better care for older adults with heart failure in the future.

Eligibility criteria
- Inclusion Criteria:
- • Person aged 65 or over.
- • Living at home
- • Moderately dependent, defined as GIR 3, 4 or 5.
- • Be or have been hospitalized in the last 30 days for heart failure
- • Have a relative volunteer to enter the PRESAGE questionnaire at least twice a week
- • Agreeing to take part in the study, or not objecting to the study being carried out, and whose relative agrees to take part.
- Exclusion Criteria:
- • People under 65 years of age
- • With severe dependency defined by a GIR 1 or 2 group.
- • Not dependent as defined by a GIR 6 group.
- • Refusing to take part in the study, or whose relative refuses to take part.
- • Older adult benefiting from the Prado heart failure program on discharge from hospital.
